Staff resistance is almost always the biggest one that gets underestimated — clinicians who've been charting a certain way for years don't just adapt overnight, and if training is rushed or shallow the whole rollout suffers for it. Data migration from legacy systems is another nightmare, especially when records are inconsistent or partially digitized. The technical side is actually the easier part compared to the change management.
